Intracranial arterial dissection is associated with subarachnoidal hemorrhage as well as ischeamic stroke. Vertebrobasilar dissection is one of the etiological factors of subarachnoidal hemorrhage in posterior system. We aimed to report a vertebrobasilar dissection that was determined in convantional serebral angiography in a 55 years old patient who was admited with a sudden onset headache and confusion and was followed up in our clinic with a posterior system subarachnoidal hemorrhage and to review the current literature. Spontaneous vertebrobasilar dissection can be followed up by conservative medical treatment unless an aneurismatic dilatation is determined in cerebral angiography.
