Aim: This study aims to demonstrate the treatment and follow-up of a complex
case that contains three teeth in a single case, involving mandibular bilateral
primary endodontic secondary periodontal combined lesions in molar teeth,
along with an apical periodontitis in a second premolar in our case archive.
Case: A 47-year-old healthy female patient reported to the Department of
Endodontics of our university with complaints of pain in right and left posterior
mandibullar region. Upon clinical and radiographic examination, tenderness in
percussion was observed in tooth 37 (FDI) and tooth 46. In tooth 46, the primary
root canal filling was observed to be insufficient in size and of low quality and
both teeth showed radiolucency in the periapical and furcation areas. Considering
the diagnosis, combined lesion (primary endodontic secondary periodontal
lesion, furcation lesion class: non-periodontitis Grade II), root canal treatment was
planned for tooth 37 and re-treatment for tooth 46. For tooth 45, retreatment was
planned due to symptomatic apical periodontitis. The endodontic treatments
were completed in multiple visits. Examination findings were recorded in 2-year
follow-ups.
Results: During the 2-year follow-up period, the radiolucency associated with teeth
37, 45, and 46 was completely resolved. Additionally, increased trabeculation in the
bone tissue, function recovery and evidence of complete healing were observed.
Conclusion: Based on the findings, it was concluded that a successful level of healing
could be achieved within two years for a complex case involving combined lesions
and apical periodontitis, following treatments performed in accordance with
traditional endodontic principles and without the need for surgical intervention.
