Abstract In previous studies, it has been shown that the “geriatric nutritional risk index” is an indicator in determining the nutritional need in patients with osteoporosis. However, in osteoporotic patients with fragility fractures, it has not been investigated whether this indicator has an effect on the fracture. The aim of this study is to investigate whether the “geriatric nutritional risk index” is different in osteoporotic patients with and without fragility fractures. The file records of female patients, aged 60 years and over, who were registered to the "Osteoporosis Clinic" within the part of Physical Medicine and Rehabilitation Outpatient Service, between September 2022 and July 2023, and diagnosed with "osteoporosis", were reviewed retrospectively. Patients with secondary osteoporosis, using steroids, having any comorbidity that may cause cancer or malnutrition, or using any protein-containing drug were excluded and a total of 48 patients were included in this study. Twenty-four patients with fragility fractures according to radiography and file examinations were accepted as the 1st group. Twenty-four patients who did not have any fragility fractures were accepted as 2nd group. Bone mineral densitometry, radiographic datas, laboratory parameters including albumin, calcium, alkaline phosphatase, phosphorus, parathyroid hormone, calcium/creatine ratio in spot urine, 25 hydroxy vitamin D values and FRAX values were recorded. The “geriatric nutrition risk index” was calculated using the albumin values and the height and the weight of patients. Comparison of two groups showed that, in patients with fragility fracture, the risk of FRAX major osteoporotic fracture was significantly higher (p=0.004), and the phosphorus value was significantly lower (p=0.003). Patients with fragility fractures were divided into 2 groups as those with vertebral fractures (n=11) and those with non-vertebral fractures (n=13), the corrected calcium value was significantly lower in those with non-vertebral fractures (p=0.012). Comparison of two groups showed that “geriatric nutritional risk index” and blood albumin levels were similar in both 1st and 2nd groups. According to our study results, no relationship was found between “geriatric nutritional risk index” which is one of the malnutrition indicators, and fragility fractures in geriatric osteoporotic patients.
