Isolated nonsyndromic congenital nasal anomalies are exceedingly rare and frequently undiagnosed until identified intraoperatively during rhinoplasty. Herein, we report a rare case of unilateral absence of the middle and medial crura, incidentally identified during rhinoplasty in a 29-year-old patient, and provide a detailed discussion of our surgical approach and management strategy. Meticulous documentation and systematic reporting of such anomalies are crucial for advancing scientific understanding, improving reconstructive methods, and enhancing clinical outcomes.
