Transmigration of Impacted Mandibular Canine: Innocent or not?

Abstract

INTRODUCTION

Transmigration is typically defined as the progression of a tooth, usually one that is embedded, within the jawbone crossing the midline. Although transmigration is commonly encountered in mandibular canines, its occurrence along with transmigration is rare (0.31%). It is mostly asymptomatic and is incidentally detected during routine check-ups. This case aims to present a rare complication with transmigrated impacted canine.

CASE DESCRIPTION

A 12-year-old systemically healthy patient was referred to our clinic from an with a diagnosis of impacted canine. Upon extraoral and intraoral examination, absence of tooth 43 and an extraoral fistula near the midline of the mandibule were detected. CT examination revealed a cyst formation associated with the impacted tooth and perforation associated with the cyst in the mandible. The patient's treatment proceeded surgically, involving enucleation of the cyst and extraction of the tooth. Biopsy results confirmed a dentigerous cyst. No symptoms were encountered during the follow-up period.

DISCUSSION

Various methods are recommended for the treatment of transmigrated teeth, including extraction, orthodontic guidance, and transplantation. Due to the presence of pathologies such as infection, ankylosis, cysts, or tumors associated with impacted canines, extraction may be indicated. Studies investigating the correlation between Ki-67 and p53 gene expression rates and radiological measurements in cystic tumor groups associated with dentigerous cysts have found a correlation between the progression of the cyst and the increase in markers with progression.

CONCLUSION/CLINICAL SIGNIFICANCE

Given the potential for complicated outcomes associated with possible pathologies related to transmigrated impacted canines, patients with this diagnosis should undergo appropriate treatment protocols promptly.
