Ischemic stroke is a major cause of morbidity and mortality. Recurrent ischemic events are partially preventable and huge number of event base medical treatments is present in the literature. We basically review of the pathogenesis of atherothombosis and mechanisms of the drugs. Current antiplatelet treatments are not seemed to have magnificent differences regarding their efficacy. Drug treatments must be chosen not only their efficacy but also their adverse effects.
