Diyabetik Ayakta Fizyoterapi Yaklaşımlarının Duyu Bozuklukları Üzerindeki Etkisi

Özet

Bu çalışma Osmangazi Üniversitesi Sağlık Hizmetleri Meslek Yüksekokulu Protez- Ortez ve Biyomekanik Ünitesi, Osmangazi Üniversitesi Tıp Fakültesi Endokrinoloji Bilim Dalı ve Hacettepe Üniversitesi Fizik Tedavi ve Rehabilitasyon Yüksekokulu’nda diyabetik ayakta fizyoterapi yaklaşımlarının duyu bozuklukları üzerindeki etkisini araştırmak amacı ile yapılmıştır. Çalışmaya Wagner skalasına göre 0. derecede tanımlanan (ayak cildi sağlam ve diyabete bağlı ülserasyonlar henüz gelişmemiş ) 34 olgu dahil edilmiştir. 17 olguya ayak problemlerine göre uygun ortotik destek, egzersiz programı ve diyabetik ayağa yönelik ayak bakımı eğitimi uygulanmıştır.
Sadece ortotik destek ve diyabetik ayağa yönelik ayak bakımı eğitimi verilen 17 olgu ise kontrol grubu olarak alınmıştır. Her iki grup olgu; 0, 6 ve 12. haftalarda duyu bozuklukları yönünden değerlendirilmiş; çalışma grubu olguların değerlendirme sonuçlarında tedavi öncesine göre, tedavi sonrası değerlerdeki iyileşme istatistiksel olarak anlamlı bulunmuştur (p<0.05). Sıcak-soğuk, sivri-künt, hafif dokunma, derin basınç ve koruyucu duyulardaki artışın kontrol grubuna göre daha belirgin olduğu gözlenmiştir. Çalışmanın sonunda diyabetik hastaların duyu seviyesinin uygun fizyoterapi ve rehabilitasyon yaklaşımları ile geliştirilebileceği belirlenmiştir.

THE EFFECT OF PHYSIOTHERAPY APPROACHES ON THE SENSORY IMPAIRMENTS IN DIABETIC FOOT This study is performed in the Vocational Health Sciences School of Osmangazi University, Endocrinology Department of Osmangazi üniversity Medical Faculty and in the School of Physiotherapy Rehabilitation at Hacettepe üniversity. The purpose of the study 5vas to investigate the effect of physiotherapy approaches on the sensory impairments in diabetic foot. 34 patients belonging to 0 degree of Wagner’s scale participated in the study. Orthotic supports, exercise programme and foot çare training were given to 17 patients in the study group. Orthotic support and training on foot çare were applied to the control group, also consisting of 17 patients. Both groups were evaluated in the beginning, in the 6"’ and at the end of 12 weeks. Post treatment sensory evaluations results were found to be signifıcantly improved when compared with pretreatment data in study group (p<0.05). The more obvious improvement is seen in the hotcold, pin-prick, light touch, deep touch and preventative sensations in favour of the study group. Consequently it was determined that the sensation level of the diabetic patients could be improved by the appropriate physiotherapy rehabilitation approaches.
